History of Thriving Weld

2007

Starting in 2007, the Weld County Department of Public Health and Environment began doing their CHA (Community Health Assessment) and CHIP (Community Health Improvement Plan), prior to the Public Health Act of 2008 requiring the process for local public health agencies.

2011

In 2011, Thriving Weld was initiated by the original backbone organizations – Weld County Department of Public Health and Environment, North Colorado Health Alliance, and North Range Behavioral Health. The first series of community meetings to identify the initial health priority areas were held.

  • Active Listening
  • Healthy Mind and Spirit

2012

Introduced and adopted a collective impact approach. Thriving Weld began developing the shared measurement system by identifying performance measures and potential participant outcomes.

The original mission was developed this year: To share responsibility for the community’s collective impact effort to make Weld County the healthiest county in the healthiest state in the US.

2014

The original Thriving Weld Dashboard and website was launched. Thriving Weld adopts its current name!

United Way of Weld County becomes a backbone organization of Thriving Weld joining the original three backbone organizations.

2017

Thriving Weld adopts a new vision statement: Make Weld County the best place to live, learn, work, and play by working toward “health for all people” through a safe, clean, health-promoting built environment.

The first ever Thriving Weld Summit was held!

2022

CHIP process was undertaken for the 4th time with new health priority areas identified by partner organizations. Began development of new mission, vision, and core values of Thriving Weld.

  • Mental Health
  • Housing

2023

Beginning of Acts of Connection initiative to address loneliness and social isolation in Weld County.

Today

The Weld County Department of Public Health Environment conducted its Community Health Survey in 2025. Work on Weld County’s next CHIP will begin in 2026 which  will lead Thriving Weld’s future efforts and priority areas.
