Course/ Class Description
This health program includes 10 visits over a one-year period. At visit 1, there will be a baseline screening & health survey. Visits 2-7 focus on a 6-week class that meets once a week and covers topics including nutrition, exercise, and mental health coaching. Visit 8 includes a 3-month follow-up. Visit 9 is a 6-month follow-up. The final visit is the 12 -month follow-up.

For anyone wanting to be proactive with their family’s health or if you or a family member has health concerns that may increase your risk of hearth disease. Your entire family, age 9 or older, is welcome to participate in the program.

If you have prediabetes, the Heart of Weld Diabetes Prevention Program can help you improve your overall health by losing weight, eating better, and being more active. It will be available to all eligible Weld County employees and offered at the Weld County Department of Public Health over the lunch period.
You’ll get:
- A proven program to prevent or delay type 2 diabetes, led by a trained lifestyle coach
- A CDC-approved curriculum with techniques to improve food choices and increase physical activity to help you lose weight
- A full year of support with weekly 1-hour sessions for up to 6 months and then monthly sessions for the rest of the year
- Several built-in incentives and prizes throughout the year to keep you motivated and on track
- Support from others like you as you learn new skills for healthy living
